Finasteride after a hair transplant
Clinics often suggest finasteride after a hair transplant. Its UK leaflet warns sexual problems can continue after stopping. Patients call this PFS.

Hair transplant clinics often suggest finasteride to protect the hair that wasn’t moved. It’s the same tablet with the same UK leaflet, which warns that some sexual side effects can continue after stopping, a condition known as post-finasteride syndrome, or PFS.
PFS isn’t only about sex. Criteria proposed by doctors and researchers in 2022 also describe problems with thinking, depression and suicidal thoughts, which can happen without any sexual change. If finasteride comes with your aftercare plan, read the leaflet before your first tablet, wherever the clinic is.
The story in 3 lines
- Oral finasteride was one of the three non-surgical treatments that members of a hair restoration surgeons' society prescribed most in 2021 (ISHRS census).
- The UK leaflet lists erection, sex drive and ejaculation problems that persisted after stopping, under "Frequency unknown".
- A trial of finasteride around a transplant ran only while the men took it (2005 trial). Patients call lasting effects PFS.

Chirag did his homework before he started finasteride. His story has no transplant in it, but it starts with the same thing: reassurance. He asked friends who’d taken it without problems, and watched doctors online who played down PFS. In this short clip, listen for how much that reassurance shaped his choice.
That reassurance, he says, is part of why he started. He stopped when sexual symptoms began, tried again at a much lower dose, and stopped again. Months later came what he calls a crash, like being “hit by a bus”. Several doctors, he says, didn’t believe it was linked to the tablets.
Why do hair transplant clinics suggest finasteride?
To protect the hair around the new grafts. A transplant moves hair from the back of the head to thinning patches, the NHS explains. The hair that stays where it was can keep thinning.
Hair restoration surgeons prescribe it widely. In their society’s 2022 member survey, oral finasteride was one of the three most prescribed non-surgical treatments in 2021.
One clinic in Turkey says something similar in its guide to finasteride after a transplant: it is “generally recommended to help maintain results”, though “not considered essential for every patient”.
Does finasteride help after a hair transplant?
In a 2005 trial, the hair around the grafts did better on finasteride. The 79 men in it took finasteride or a dummy pill from four weeks before their transplant until 48 weeks after.
The trial measured hair. A visible increase in hair at the front and top of the head showed in about 9 in 10 men on the tablet, against about 7 in 10 on the dummy pill. The published summary calls finasteride “generally well tolerated”.
That summary covers the men only while they were taking it. It says nothing about what happened after they stopped, which is where PFS begins.

Can finasteride side effects last after stopping?
Yes, for some people, and a transplant doesn’t change the warning. It’s the same 1 mg tablet people take for hair loss without surgery.
The UK leaflet says some side effects “disappeared when treatment is stopped”. It then lists three that didn’t, including “persistent difficulty having an erection after discontinuation of treatment”. Next to each it says “Frequency unknown”: the leaflet gives no figure for how often it happens.
For some people it has lasted years after stopping. Our page for anyone worried about PFS covers what is known about risk, and what isn’t yet.
In May 2026 the MHRA, the UK medicines regulator, told prescribers to explain these risks when they prescribe it. A card in UK packs covers them; our page on finasteride warnings has its words. If a clinic abroad hands you tablets, the leaflet in the box may be another country’s. You can read the UK one online first.
What can I do?
These medicines help many people. Whether to take finasteride, before or after surgery, is a decision for you and your prescriber.
If you’re already taking it, the leaflet says to stop and talk to your doctor if you get depression or suicidal thoughts. If you’re struggling, the Samaritans are free on 116 123 in the UK and Ireland, day or night.
If side effects haven’t lifted since your last tablet, tell your GP. Many people say it took time to be taken seriously; Chirag says several doctors didn’t believe him. Bring the leaflet line. You can report it without a doctor agreeing.
Taking it to your GP
An opening line you can use: "I've had changes since stopping finasteride, and the leaflet says some can continue after stopping."
- Your dates: "Finasteride 1 mg from March 2025, started four weeks before a hair transplant, last tablet January 2026."
- What changed, in your words: "less feeling", "no sex drive", "can't think clearly", "low mood".
- How things were before you started.
- Could anything else explain it, and what would you check?
- Can this be recorded as a suspected side effect?
Report it: it helps everyone
A doctor’s note helps you. A report to the medicines regulator helps everyone else: it is how patterns get noticed.
- No proof needed. A suspicion is enough, even years after stopping.
- Where: the Yellow Card scheme in the UK, MedWatch in the US.
- Use the right words. If symptoms continued after stopping, write Post 5-alpha-reductase inhibitor syndrome (MedDRA 10082430).

What else do people ask?
Do I need finasteride after a hair transplant? The Turkish clinic’s guide says it isn’t a must for every patient. Whether it fits you is something to talk through with whoever prescribes it, with the leaflet in front of you.
Will my transplanted hair fall out if I stop? The clinic’s guide says the moved hair resists the hormone finasteride blocks. It also says the tablet protects the hair around the grafts only while you take it. Our page on stopping finasteride covers both hair and side effects.
Is topical finasteride different? It’s put on the scalp, and less of it reaches the blood, but not none. The FDA has had reports of sexual problems that continued after people stopped. Our page on topical finasteride sets out what its leaflet says.
Chirag started after friends and doctors online reassured him. If you’re about to start finasteride with your transplant, read the leaflet first, and if it has left you with PFS, report it.
