What the conditions are

PFS, PSSD and PAS describe sexual, cognitive and emotional symptoms that continue after finasteride, SSRIs or isotretinoin have been discontinued. They affect a minority of patients, but for those affected the symptoms can be long-lasting.

Regulatory recognition

The FDA required finasteride labelling to note that sexual side effects may persist after discontinuation (2011). The EMA required the same for SSRIs and SNRIs (2019). MedDRA added Post-Finasteride Syndrome as a coded term in 2021.

An awareness gap, not a dispute

A 2024 Harvard Medical School survey found that 18% of dermatologists considered persistent effects possible. These conditions are uncommon and are not yet a routine part of medical training — which is where better information helps most.

What the biology suggests

A peer-reviewed study found expression differences across 3,764 genes in PFS patients, and animal research has identified distinct brain alterations — findings that are difficult to attribute to expectation alone.

Reports on record

WHO VigiBase holds 21,287 finasteride adverse event reports from 153 countries; FDA FAERS holds 11,557. These databases record reports, not confirmed causal links — but they show the scale of what is being reported.

Why the numbers are likely low

Adverse event reporting is voluntary, and the FDA estimates that only 1–10% of reactions are ever reported. Establishing the true prevalence of these conditions remains an open research question.
